Projects for Assistance in Transition from
Homelessness (PATH)
The Stewart B. Mckinney Homeless Assistance Act of 1990 authorized a Federal grant program to deal the needs of people who are homeless and have serious mental health illnesses. The program -- known as PROJECTS FOR ASSISTANCE IN TRANSITION from HOMELESNESS (PATH) - funds community-based outreach, mental health, substance abuse, case management and other support services, as well as a limited set of housing services. The PATH program reaches some of the most vulnerable member so of society.
It delivers essential services, leverages significant State and local resources, and marshals a creative network of human services organizations to improve the mental health and well being of people who are homeless and have mental illness.
Among the services eligible under the PATH program are: outreach services, screening and diagnostic services, habilitation and rehabilitation services, community mental health services, alcohol or drug treatment services, case management, supervisory services in residential settings and a limited set of housing services and services to help client access housing resources. TCCSC’S PATH program works in collaboration with New Image Shelter in Los Angeles. We provide the following services to clients at New Image Shelter: outreach, case management and linkage to community resources. We assess people and refer them to TCCSC if they are eligible for the Senior Wellness Support Program or the TAY program under FSP.
