Wraparound

Many times youth and their families have complicated multi-dimensional needs that cross agency boundaries. For that reason, the need to provide individualized, comprehensive services that meet their strengths and needs across home, school and community is great. The ultimate goal in Wraparound is for the child to live an independent, fulfilling, law-abiding and constructive life in the community with minimal special supports.

The goal of Wraparound is to provide appropriate, coordinated, quality, and cost-effective services in a child's home or community. In the provision of Wraparound Services, TCCSC takes a collaborative, team-based approach to service delivery and family support planning. As such, TCCSC coordinates agency, division, family, and community efforts to most appropriately serve the multiple and unique needs of children and their families.

Wraparound Services are designed to assist families in keeping youth in their homes and communities and out of residential institutions. Services are provided to children and families who may be struggling to stay safe and stay together while, at the same time, maintain everyday life and functioning.

In order to address the diverse needs of the target population, Wraparound Services enable youth and families to receive the level and intensity of services deemed appropriate based upon their identified strengths and needs. The intensity of services offered to a youth is dependent upon the initial assessment and Child and Family Support Team meeting that involves service providers, the youth and family, systems persons and others in the natural support system. The initial assessment process will identify strengths/needs in multiple domains including: education, safety issues in the home and community, family, self, and psychosocial needs.
